Finding the right specialized care for your child can be a daunting task. In California, Regional Centers provide qualified families with respite services to help them get the support they need — and as of July 2021, there have been some changes in the variety of options available. We talked to our fellow Undivided families, including Public Benefits Specialist Lisa Concoff Kronbeck, Care Navigator Lisa Carey, and Independent Facilitator and Person-Centered Planner Carla Lehmann, to help us understand what’s possible. We’ve also put together a list of resources that can help you find the right specialized care for your child. 

If you already receive respite hours from Regional Center, great! If you’re not yet set up with a Regional Center service coordinator, you can learn more about these services at the links provided. 

 

Traditional Respite Services

Your local Regional Center works with respite care agencies that can help you find a respite worker. Here’s what you need to know:

  • These agency respite workers are bound to an in-home only service and paid minimum wage. If you need care outside your home, you may ask for an exemption. 
     
  • Each Regional Center holds different rules for children with medical conditions. Lisa Concoff Kronbeck advises that if your child has a specialized medical condition, it’s important to find out what type of intervention is allowed by the respite worker. For example, some centers require respite workers to have licensed vocational nurse (LVN) training to be allowed to change a G-tube or intervene during a seizure. Other Regional Centers dictate that their respite workers cannot practice these interventions while some allow parents to train workers in this area. 
     
  • Remember that if you don’t find a respite care worker to be a good fit, you can request different care providers at any time. 
     
  • During the Covid pandemic, some Regional Centers have temporarily delayed the requirement of CPR and first aid certification. Other centers require that these certifications be done through an online program.  
     

Parent Conversion

Another option for respite care is parent conversion, also known as parent/family choice. What this means is that the parent finds their own caregiver and then helps that caregiver register with a respite agency in order to bring them on through Regional Center.

  • Lisa Carey suggests that parents “go about this the same way they would finding a babysitter. They can use their family members, friends, parents’ groups on social media, or real-life connections. They can also use Care.com or colleges like CSUN.” Carey adds that she prefers parent conversion over agency-assigned as it allows the parents to be in more of a supervisor role, dictating what respite care workers can and cannot do. You can look at colleges in your area that offer programs to prepare students to work with children with disabilities, such as autism-centered studies, special education, or ASL. We’ve included several in this report.
     
  • As veteran respite care finders know all too well, finding a qualified and trustworthy person to care for your child can be a major task. Searching for someone willing to work for the state respite care standard of $15 per hour presents an additional challenge. Several Undivided parents shared with us that to offset this low pay, they make up the difference in rate by privately hiring the caregiver for extra work outside respite care hours and paying their own determined (much higher) rate. (Please note that this is only possible when hiring through the parent conversion method. If you choose to have an agency provide you with a respite worker, you cannot privately hire or offer to offset low wages.)
     

Self-Determination

As of July, Regional Center’s Self-Determination Program (SDP) is now available to all qualifying clients. SDP allows parents to create an individualized plan that’s personalized to their child’s strengths and needs, and it can be particularly useful for families who don’t live near Regional Center vendors or who don’t have local agencies that can provide care specific to their child’s needs.

SDP is a voluntary route that Carla Lehmann describes as offering more flexibility, freedom of control, and better quality of services. Lehmann assures us that it will not cost more money, but it will initially involve more work. With this approach, you have control over your budget and can put money toward services and resources that are more effective for you and your child. Under Self-Determination, the respite care provider of your choice is not bound to the in-home-only, minimum-wage rules of the traditional respite agency. Since you are creating the budget, you are allowed to choose the type of home service and dictate the rate you pay, giving you the option to pay more or receive more respite hours for the same price that Regional Center would have paid a respite agency. (Lehmann explains that SDP eliminates going through the respite agency, which takes a cut of the budget.) By eliminating the respite agency, this removes rules and restrictions that Regional Center abides by. Your respite provider is able to drive your child to different locations, such as a socialization class, which allows you to have respite time while your child is busy and entertained.

  Specialized Nanny Placement Agencies

This agency places a wide range of college-educated service providers, such as full-time nannies, nannies for children with disabilities, tutors, personal assistants, and many more. All of the nannies they match with families have years of experience or are recent college graduates. In addition, all nannies go through a background check, drug screen, and reference review. This agency has experience placing nannies with families who have children with autism, cerebral palsy, Down syndrome, and other disabilities. 

  • Process:
     
    • Families complete an application describing their needs.
       
    • The agency will send a list of resumes from well-matched candidates.
       
    • Interviews will be set up with the candidates and the family.
       
    • There is an option to have a trial period for potential candidates (up to 24 hours over the course of a week). 
       
    • Please keep in mind that if you are only looking for part-time help, the majority of their caregivers require a minimum of 20 hours per week and start at $25 per hour.
       
    • Once a family is ready to hire, they will be sent a Nanny Family Agreement. 
       
  • Cost: 
     
    • There are no registration or start-up fees. Parents can interview as many candidates as necessary to find the right fit, and they are able to have a trial period that includes paying the nanny for their time. If a family decides to have a trial period extended beyond 24 hours, they will need to pay a nonrefundable deposit of $500 to the agency. 
       
    • A one-time placement fee (which is 15% of the nanny’s annual gross salary) is due at the time of hire. There are no recurring fees. 
       
    • The agency will provide families with the average salary range in their area, and families can decide if they want to pay a salary or hourly rate. Most rates average $25 per hour but can be up to $35 per hour depending on speciality. 
       
    • Families will be matched with candidates who fit their budget. 
       

Westside Nannies

This agency places caregivers in a wide variety of areas, including full-time and part-time nannies, private educators, newborn specialists, and those who have experience working with kids who have disabilities. These nannies are experienced in delivering medication, CPR-certified, and willing to receive specialized training. In addition, they will work collaboratively with the child’s therapists and educators to create a cohesive team. The agency has matched families with nannies who have experience working with kids with mild to severe learning disabilities, physical disabilities, and social-emotional and behavioral issues. They have recently placed nannies with families who have children with Rett syndrome, Down syndrome, and autism.

  • Process:
     
    • Start here by answering a few questions and setting up a consultation. The agency will ask for information about your family, schedule, and specific needs. 
       
    • Based on this information, the agency will find the best-matched candidates and email resumes to the family for consideration. 
       
    • Once a family chooses their preferred candidates, the agency will schedule interviews.
       
    • After a family selects a nanny, the agency walks them through the hiring process and employer responsibilities. 
       
  • Cost:
     
    • For Los Angeles–based families, the agency requires a placement fee, which is 18% of the nanny’s total gross annual compensation. This is paid by the family and is due upon hire. Nationwide and private educator searches incur a placement fee of 25%. 
       
    • Gross hourly rates range from $25 to $40 depending on specialty. Most professional nannies also receive a two-week paid vacation, paid holidays/sick days, health insurance stipend, and mileage reimbursement for on-the-job transportation.
       

Educated Nannies

Founded by a mother who knows the value of good care providers, this agency seeks to ease the stress of finding a professional nanny by interviewing every nanny and contacting their references. Upon hiring, background checks are conducted, and the nanny is registered with TrustLine. The agency places live-in and live-out full-time and part-time nannies. They have experience placing nannies who have ABA training and experience with children who have disabilities. In addition, their nannies are technologically savvy and ready to support children with distance learning. Speciality services include babysitting, private educators, a newborn care class, and Mommy and Me classes.

  • Process:
     
    • After completing the basic intake form, founder Ryan Jordan will initiate a video call to learn exactly what the family is looking for. 
       
    • The agency will interview all local candidates and call each reference on the family’s behalf. 
       
    • Parents are sent candidate resumes for review, and the agency schedules interviews with the family and prospective nannies. 
       
    • A five-day trial period is granted to ensure that both the family and the nanny are comfortable. 
       
  • Cost:
     
    • View their services, fees, and compensation information on their website. 
       
    • Every service starts with a $500 registration fee based on the program, and the placement fee is 18% of the nanny’s gross annual income. Specialized searches or placement outside of California are 20%.
       
    • In addition to the matching process, the family will receive benefits such as complimentary membership to their babysitting program for one year, an employment agreement template, and a prorated fee of 50% toward the next search if employment ends before six months. 
       
    • Full-time nanny rates average $30 to $40 per hour. Full-time nannies receive two weeks of paid vacation, minimum of 48 hours of paid sick time, paid holidays, and federal mileage reimbursement rate if they are using their own vehicle. 
       


  Nanny and Sitter Services

Joshin is a caregiver networking website created to provide quality care for those with disabilities. Joshin aims to give families a way to find specialized caregivers for their unique needs, such as sitters, nannies, and tutors, support for an adult with developmental disabilities, or a PCA/CNA-trained provider for a child with medical complexities. The company requires each caregiver to go through an extensive safety check before they are allowed to join their network. This includes a background check, disability experience with a minimum of six months caring for people with specific care needs, a personal review of each caregiver, an individual care plan, and a social media check. 

Note: Joshin is an early stage company and is working to expand to a new community of individuals, families, and caregivers. If you cannot find anyone in your specific area you feel would be a proper match, send them an email to let them know where you are, and they can follow up with steps to help. 

  • Process:
     
    • Create a Care Plan for your child’s health and personal information so that a future caregiver can provide the best care possible. 
       
    • You may begin your search for a caregiver by browsing the caregiver profiles and their reviews in your area. You also have the ability to post your care needs as a job (with unlimited job post editing) to all caregivers in your area at no extra cost.
       
    • Once you find caregivers, you may message and interview the ones you feel could be the best match. 
       
    • Once you have selected a caregiver, you send them a Care Date request. Joshin allows you to manage the scheduling and all payments with your selected caregiver through their website. 
       
  • Cost:
     
    • Membership rate: Visit the website to learn what your membership covers and includes. You can pay for Joshin through a subscription (out of pocket), through your Self-Determination Program budget, or through your self-directed Medi-Cal waiver budget. (You would need to add Joshin to your service plan. You can do this by reaching out to your support planner or support broker at your Medi-Cal plan and letting them know you would like to add Joshin for recruitment services.)
       
      • One-month membership: $29.99

    • Caregiver rate: You set your own hourly rate based on factors such as level of care, siblings, and location. Average rates are $16 to $30 per hour.

        • For families who are paying out of pocket, the caregiver will get paid within a few days of the Care Date via Stripe, the system used by Joshin.
           
        • If you are a member who uses a self-directed Medicaid budget, or you have respite and/or specialized supervision hours awarded by Regional Center in your Individual Program Plan (IPP), you will not be charged for the Care Date. Instead, you complete a timesheet through the agency Regional Center assigned to you. Your Joshin caregiver should have already completed the hiring process with your agency and will be paid on their regularly scheduled payroll dates.

Serving Orange County, Long Beach, and Los Angeles with nannies and sitters (either at home or in local hotels), this agency will personally match each family with a nanny or sitter who meets their individual needs. Sitters will arrive with an activity bag full of toys, games, and books to make sure your kiddos have fun. The sitters are all CPR- and first aid-certified and have a minimum of three years of child care experience. The agency understands the needs of families with children who have disabilities, but they ask that families be descriptive and clear about the child’s specific needs so that they can provide the best match. 

  • Process: 
     
    • For families needing a nanny:

    • For families needing a sitter:

This website offers caregivers including nannies, babysitters, and housekeepers. The caregivers undergo federal and county criminal background checks along with Social Security number tracing, national sex offender public website search, and multi-jurisdictional criminal database investigation. Parents are encouraged to conduct their own background checks for extra caution. Their website has a section specifically for people with disabilities. When conducting a search, a list of disabilities appears on the screen for the applicant to choose from.

  • Process:
     
    • Create a login account.
       
    • Parents can post a job and include:

This is not an agency but an online meeting venue/job board; the site is not responsible for employing anyone. Parents can post jobs and view available babysitters for infants through middle school-aged children. The sitters also offer virtual child care, where children are engaged digitally for a short period of time. The sessions can be used to help kids with schoolwork or facilitate an interactive game; it’s not a replacement for physical care. There is a section for children with disabilities using keyword search. Parents are encouraged to conduct their own background checks, which can be ordered directly from the sitter’s profile.

Like Care.com, you can search for caregivers with experience working with kids who have disabilities.

    • Post a job and search for babysitters and nannies for free; you do not pay UrbanSitter until you contact a sitter.
       
    • Becoming a member allows you to book and schedule sitters, set reminders, and find contact details. 
       
    • Their babysitting app allows parents to easily find, call, message, and book sitters on the go.

 Skilled Nursing and Home Care Companion Agencies

Serving the Santa Monica and Torrance areas, BrightStar provides care for children with disabilities and unique care requirements. The agency has experienced care professionals who specialize in administering in-home care, and they can provide comprehensive case management and support for children with disabilities such as autism, spina bifida, amyotrophic lateral sclerosis (ALS), and cerebral palsy. The three main home care service options they provide are companion care, personal care, and skilled nursing care. Companion care includes sitter services and meal planning and scheduling, and personal care includes feeding, toileting, bathing, and ambulation. As part of their guaranteed compatibility program, all caregivers are rigorously screened and have at least one year of experience; most have advanced training. 

  • Process: 
     
    • Contact BrightStar and arrange a time for a complimentary initial personal assessment. 
       
    • BrightStar’s Director of Nursing creates each client’s personalized plan of care and oversees its delivery by their professional care staff.
       
    • BrightStar will arrange for services ranging from a few hours a week to around-the-clock care.
       
  • Cost: 
     
    • Initial assessment and conversation with a BrightStar Registered Nurse is free.
       
    • BrightStar offers several levels of care, and costs are determined based on each client’s needs, including level of service and hours needed per week. 
       
    • Find the nearest BrightStar Care office and have an intake coordinator assess your situation. They will be more than happy to provide you a quote and set up a payment plan that works for you.
       
    • No subscription or membership fees.
       
    • Average cost of home care:
       
      • Companion care and personal care: Average rate is $32 per hour. 
         
      • Skilled nursing average rates start at $55 per hour and $110 per hour for a registered nurse. 
         
    • Medicare does not pay for this type of care; however, many long-term care policies will reimburse for these kinds of services. BrightStar is happy to work with your insurance company to make that determination. They will also assist you by billing the insurance company directly.   Low-Cost, No-Cost, and Subsidized Child Care

Connections for Children is a non-profit child care resource and referral agency serving parents, child care providers, and educators for the Westside and South Bay communities. Their mission is to help find quality child care regardless of income. In addition, they offer free information and resources for children with disabilities and recommend that families call them directly for support. For low-income families needing child care, visit their subsidized programs page for more information. 

  • Process:
     
    • They provide parents with a list of providers from their referral database. They believe that parents are best able to evaluate and select the right child care for their children, so they do not recommend any specific provider. Instead, they suggest that parents visit any child care facility they may be considering and make their decision carefully.
       
    • There are three ways to get information on selecting child care providers:
       
      • Call the resource and referral counselors at (310) 452-3325.
         
      • Visit their offices (walk-ins welcome).
         
      • Conduct a free online search for a provider in your area.
         
  • Cost:
     
    • Prices range depending on financial eligibility. The agency helps find subsidies for those in need.
       
    • Funding for child care subsidies can be provided through federal, state, and local funding programs.
       
    • They provide a list of low-cost and no-cost programs.
       


  College Tutors Resource Sites

CSUN uses Handshake, an online database where employers can post jobs and/or internships for undergraduate and graduate students (including alumni) as their online job board. LMU Babysitters

Loyola Marymount University (LMU) students created this Facebook group to help parents find babysitters, tutors, and coaches from within their school. It began in 2013 and has grown mainly from referrals made by parents and word of mouth on campus. The group has more than 1,500 members and “continues to be a trustworthy community and resource maintained by and for students and families.” They do not have the credentials to vet babysitters themselves, so the vetting process is done between parents and students through conversation or meeting in person before the first babysitting session. The babysitters typically live on or near campus (LMU, Westchester, Playa Vista), and students are able to travel to surrounding areas. The areas served include Inglewood, Manhattan Beach, Culver City, Santa Monica, and other cities in the South Bay area. While not limited to these neighborhoods, these are the most common areas they see in their posts.

  • Process:
     
    • Fill out their form to tell them more about your family. 
       
    • Make a one-time payment of $15 through Venmo or Paypal to @LMUBabysitters or lmubabysitters@gmail.com
       
    • Make a request to join the Facebook group, and they will add you once all the steps are completed. 
       
    • Use the Facebook group to create a job posting or look through the student posts and respond directly if interested.
       
    • Book the student who’s the best match for your family’s needs.

The LMU Community Connections job board connects students with people looking to privately hire them. Posts are active for 60 days and will be shared with all departments on campus. Students are encouraged to contact the poster directly. When posting for a babysitter, nanny, tutor, or homework helper, explain the background or experience needed for the position, such as the academic subjects the student will focus on.  

  • Process: 
     
    • Complete a job posting request. 
       
    • Within the description of the job, be prepared to include:

Uloop 

The Uloop posting job board connects students from selected universities with people looking to privately hire them. You may post for a variety of positions at multiple local colleges, for example at California State University Northridge (CSUN), for such services as babysitting, tutoring, and nannies. By posting with Uloop, parents and employers are able to view quality student applicants, post for the desired position at the college of their choice and surrounding schools, and manage the listings in a single location (this includes distribution on Uloop and local college newspaper websites).

  • Process: 
     
    • Complete a job posting request, which will stay up for 30 days.
       
    • Your listing will be posted to every school within 50 miles.
       
  • Cost:
     
    • A monthly subscription of $125. 
    • The subscription will automatically renew for successive one-month periods until you cancel your job listing. You have the option to cancel at any time.
       


  Respite Companies Vendored with Regional Centers

Services such as respite and specialized supervision must be requested through the Individual Program Plan (IPP) or the Individual Family Service Program Plan (IFSP) process. Your Service Coordinator will be the one to discuss access to this service. 

When awarded respite and/or specialized supervision in the IPP or IFSP, you as the client will choose either parent conversion (also called parent or family choice) or an agency contracted with your Regional Center. 

  • Agency: If you choose an agency, then the agency will choose a respite provider. 
     
  • Parent Conversion: Parent conversion is when you choose your own caregiver. This can be a close family member or friend who does not reside in the same residence as the client receiving the services. Many Regional Center families prefer this service because they feel comfortable leaving their loved one with someone they already know and are familiar with. If you select parent conversion, then an agency will be assigned to you, and you will need to find your own caregiver and then send them to the agency to get hired. 

Once you choose agency or parent conversion, you are ready to begin.

  • Process:
     
    • The agency becomes the legal employer, subjecting the care provider to the same hiring scrutiny as their other staff, including criminal background and reference checks, CPR, and first aid certification. They pay the worker weekly after reviewing timecards against the Regional Center’s authorization.
       
    • The agency will calculate, withhold, and file applicable payroll taxes.
       
    • They bond and insure their workers with both liability and workers’ compensation insurance.
       
    • Along with the agency, you ensure that the care provider and your family set schedules that work best for you and your care provider within the limits (quarterly, monthly, weekly, or daily) authorized by the Regional Center.
       
  • Cost:

Accredited Nursing  

Accredited Nursing provides home care, home health care, and Regional Center Services throughout Southern California. This includes “respite care in the home so that other family members can get rest and financial management services in conjunction with Regional Center institutions.” In 2001, Accredited introduced the Parent Voucher Conversion Program as a vendor of the North Los Angeles Regional Center. Since that time, their relationship with Regional Centers has continued to expand. They now service 18 of the state’s 21 Regional Centers.
 

24 Hour Homecare 

24 Hour Homecare is contracted with multiple Regional Centers to provide in-home respite, specialized supervision care, and more. The purpose of 24 Hour is rooted in allowing individuals with “intellectual and developmental disabilities to continue a full, active, and healthy way of life by providing premier non-medical homecare.” This includes respite care for primary caregivers to take a break from usual daily responsibilities to allow them an opportunity to take care of themselves.  

 

Cambrian Home Care 

Cambrian is a privately owned company that has been providing care for over 25 years to Southern California families. They offer a variety of services, which include in-home respite care for those with developmental disabilities. They are contracted with many Regional Centers to provide respite care throughout Southern California including Harbor, East Los Angeles, South Central Los Angeles, Lanterman, San Gabriel/Pomona, Orange County, North Los Angeles, Westside, Inland, San Diego, Alta, and San Andreas. Cambrian’s respite providers are able to offer caregivers temporary relief in giving them time to leave the home for other obligations or just have time for themselves. During respite care, the providers may provide social interactions with games or reading, hygiene care, behavioral activities, routines such as homework, and sibling care for one or two siblings in the home while caring for the client. 

 

Tender Touch Homecare 

Tender Touch specializes in offering families of Southern California Respite and Personal Assistant Services for children and adults with autism, Down syndrome, cerebral palsy, global delays, and other disabilities. As a respite care agency, they are contracted to serve local Regional Centers and offer quality respite care for families. They serve Los Angeles, San Fernando, Santa Clarita, Antelope, and Ventura counties. Tender Touch screens and provides training for all of their caregivers before they begin providing Respite or Personal Assistant Services. They also have Spanish-speaking staff available.

 

Aveanna Healthcare 

Aveanna Healthcare joined with Premier Healthcare Services in 2018 to become Aveanna Healthcare Support Services. They are contracted with all 21 of California’s Regional Centers to provide a variety of valuable resources such as respite, personal assistance, daycare, and participant-directed services throughout the state. In addition, Aveanna is participating in the Self-Determination Program as a Financial Management Service (FMS).  They understand the dedication full-time care takes and the need for short-term relief, so they are committed to offering compassionate and trusted respite providers. In addition, they offer a prompt hiring process and accurate payments to your respite care providers.
